Incidents are often related to the "human failure" or respectively the "human factor" as the main contributing factor. Such terms describe the symptom of inadequate system-design related to human performance criteria without properly analysing these. Such understanding results in errorneous conclusions about the role of the human in error promulgation and hence in the human contribution to a reliable system design. This standard describes the generic process how to analyse events regarding human behaviour and provides opportunities to collect information for system-design from analyses of unwanted events. The standard describes how causes for human failures can be identified and how appropriate mitigations can be derived for system design.
